Standard of Care: S42.0 Fracture of shoulder and upper arm

• Initial Assessment:

• History: Detailed description of the injury mechanism, pain location, radiation, severity, exacerbating/alleviating factors, relevant medical history, and prior shoulder issues.

• Physical Exam: Swelling, deformity, bruising, tenderness, range of motion testing (active and passive), neurovascular assessment (sensation, motor function, distal pulses).

• Imaging: X-rays (typically a minimum of 3 views: AP, lateral, axillary) as the first line. CT or MRI may be indicated if a more complex fracture is suspected.

• Treatment Plan:

• Non-surgical:

• Immobilization with a sling, swathe, or specific brace depending on fracture location.

• Pain management: NSAIDs, acetaminophen, muscle relaxants, or short-term opioids in cases of severe pain.

• Ice application in the acute phase.

• Early referral to physical therapy for gentle range of motion exercises and progression as the fracture heals.

• Surgical:

• Indicated for displaced fractures, open fractures, severe comminuted fractures, or those affecting joint function.

• Types of surgery may include open reduction and internal fixation (ORIF), pinning, or shoulder replacement in severe cases.
