- Read the implementation
- Execute relevant tests
- Verify the behavior manually if needed
- Document pass/fail for each criterion

Acceptance Criteria Verification
What to Check
- Functional correctness - Does it do what the AC says?
- Edge cases - Does it handle unusual inputs?
- Error handling - Are errors handled gracefully?
- Tests exist - Are there tests covering the AC?
- Tests pass - Do all tests ACTUALLY pass when you run them?
CRITICAL: You MUST Actually Run Tests
Code review alone is NOT sufficient for QA. You must:
- Check the project's README or documentation for test instructions
- Install project dependencies before running tests
- Run the actual test commands for this project
- See the test output with pass/fail results
- Only report "PASSING" if tests actually passed
How to Find Test Instructions
- Check
README.mdfor test commands - Look for test config files:
pyproject.toml,package.json,Makefile,Cargo.toml, etc. - Look for a
tests/ortest/directory
If Tests Won't Run
If you get errors like "command not found" or "module not found":
- Install project dependencies (check README for instructions)
- Install the test framework if not included in dependencies
- Try again
DO NOT claim "tests_status: PASSING" based on code review! DO NOT claim tests pass if you couldn't run them!

If you find issues NOT in the acceptance criteria, create bug tasks:

Handling Missing Tests
Rule: Missing Tests = QA Failure (NOT a new task)
If a task is missing required tests, FAIL the task so the developer adds them during rework:

Why? Creating separate test tasks allows features to be marked "done" without tests. Tests should be part of the original task's Definition of Done.
Rework Cycle for Missing Tests
TASK-001: Implement login
→ Developer implements (no tests)
→ QA fails: "Missing unit tests"
→ Developer adds tests (rework)
→ QA re-tests → PASS
→ Feature is "done" WITH tests ✓
When QA SHOULD Create New Tasks
Only create new backlog tasks for issues outside the scope of the task being reviewed:

PASS requires ALL of:
- Zero test failures (not "most tests pass" - ALL must pass)

- VERIFY criterion confirmed (manually tested)
- Manual verification passed (if user-facing)
FAIL if ANY of:
- Any test fails → FAIL (specify which test)
- AC not met → FAIL (specify which AC)
- VERIFY criterion fails → FAIL (describe what didn't work)
- Manual verification fails → FAIL (describe the issue)
- Coverage too low → FAIL (current % vs required)
Bugs Found (outside AC scope)

## Failure Reasons
When setting `failure_reason`, be specific:
- ❌ "Tests fail" - Too vague
- ✅ "AC2 failed: Login form does not validate email format"
- ✅ "Test coverage is 62%, below 85% threshold"
- ✅ "AC3 not implemented: Error messages not shown to user"

QA Best Practices
- Be thorough - Test each AC explicitly
- Be specific - Give clear failure reasons
- Be fair - Only fail for legitimate issues
- Be helpful - Suggest what tests to add when failing for coverage
- Run tests - Always run the test suite
- Enforce coverage - Fail tasks without adequate tests (don't create separate test tasks)
- Create bugs only for out-of-scope issues - Don't use bugs to bypass rework
- Trust the rework cycle - Developer will fix failures, including missing tests
